In order to receive care, Guardian(s) must:
1. Sign this Requirements & Release.
2. Provide any necessary medical and allergy information. Jovie of North Texas staff cannot dispense medication. Guardian(s) must attend to all medication needs prior to or during the event.
3. Remain at the event site while the child is under the care of Jovie of North Texas and provide a mobile phone number where you can be reached in case of an emergency.
- Sign In: All Guardians must present a photo ID or driver’s license at sign-in. A tag will be placed on each child’s back with their name and Guardian's name, and a wristband will be provided if your child has an allergy.
- Pickup: Children must be picked up within 15 minutes of the event's conclusion. Only the Guardian(s) signing the child in may pick up the child. The Guardian ID must be presented and will be matched to the child tag. If a different Guardian must pick up the child, complete the “Alternate Guardian Authorized for Pickup” section at the bottom of the page. This person must also provide the Guardian tag at pickup.
- Photography: Jovie of North Texas may take photographs of the children during the event solely for the purpose of providing updates to families. In addition, Jovie of North Texas may take photographs of children and adults during the event for use in its newsletter or similar internal materials. By signing this Release Form, I acknowledge and agree to the taking of photographs for these purposes.
By signing, I, the legal guardian of the minor child(ren) submitted, on behalf of myself, my family, and my child(ren), hereby release and agree to indemnify, defend, and hold harmless AARS Educational Services, INC. DBA Jovie of North Texas, its affiliates, franchisor, subcontracted franchisees, and parent companies, and their officers, directors, sitters, employees, and agents (the “Releasees”) from any and all liability, losses, claims, damages, thefts of or damage to personal property, personal injuries (including death), costs, or expenses of any nature (“Claims”) arising from or related to the event services. This release shall not apply to the extent that such damage or injury arises.
Privacy.
Information I provide will be used only to deliver event services, safety, and logistics; it will be shared only with staff who need it for those purposes. I may request copies of these records or withdraw consent to electronic records at any time.
Electronic Records & Signature Consent.
By checking the consent box and typing my full legal name below, I agree to use electronic records and electronic signatures for this transaction. I understand my typed name, together with the date/time, constitutes my electronic signature and has the same legal effect as a handwritten signature.
